The Pressure of “Eating Clean” When Trying to Conceive

Navigating fertility health advice can quickly turn an intentional lifestyle into an overwhelming list of rules, biohacks, and restrictive diets. When trying to conceive (TTC), the pressure to optimize every meal creates high-stakes anxiety. Paradoxically, the severe stress caused by trying to control your diet can work against your reproductive goals. 

One of the most harmful phrases in wellness culture, in my opinion, is “eating clean.” This term carries an implicit moral judgment: if some foods are “clean,” all others are labeled “dirty.” Classifying everyday foods or occasional treats as dirty generates unnecessary guilt, shame, and fear.

In clinical practice, shifting language from “clean” to “nourishing,” “wholesome,” or “satisfying” removes moral weight from food choices. A truly healthy lifestyle is flexible enough to include both nutrient-dense meals and enjoyable foods like a donut without compromising health or fertility.

When it comes to healthy living, there is such a thing as too much of a good thing. Fertility nutrition requires a balanced spectrum: nourishing your body while retaining the joy and flexibility food is meant to bring. When healthy eating tilts into rigidity and obsession, it crosses the line into Orthorexia Nervosa.

Disclaimer: most of the research conducted around fertility and disordered eating – focuses on diagnosed EDs such as anorexia nervosa and bulimia nervosa. Here we explore how common disordered eating patterns can also impact fertility.

What Is Orthorexia Nervosa?

Orthorexia nervosa is an obsessive fixation on eating pure, “clean,” or perfect food. While driven by a desire for optimal health, orthorexia can impact fertility by possibly causing undernourishment, hormonal imbalances, anovulation, and chronic stress that can disrupt the reproductive system. 

While orthorexia is recognized by clinical dietitians and mental health experts, it is not yet classified as a formal diagnosis in the Diagnostic and Statistical Manual of Mental Disorders (DSM-5).

Because of the lack of formality around the term there are no clinical signs and symptoms that would relate to the “diagnosis”. However, there are common signs seen in those who relate to Orthorexia.

Common Signs of Orthorexia

  • Rigid Food Rules: Eliminating entire food groups (e.g., carbs, dairy, fats, processed items) without a medical reason.
  • Emotional Distress: Experiencing severe anxiety, guilt, or shame when eating non-qualifying foods.
  • Social Isolation: Avoiding restaurants, social gatherings, or family meals to maintain complete control over ingredients.
  • Moralizing Food: Viewing dietary adherence as a reflection of self-worth or personal discipline.
  • Compulsive Research: Spending hours every day reading labels, researching ingredients, or tracking “clean” eating trends.

How Orthorexia Can Impair Fertility and Reproductive Health

Conception depends on three nutritional pillars: adequate energy availability, sufficient micronutrients, and a properly functioning hypothalamic–pituitary–ovarian (HPO) axis. Orthorexic eating patterns can disrupt all three.

Low energy availability and functional hypothalamic amenorrhea (FHA)

When caloric intake consistently falls below what the body needs, it conserves energy by downregulating non-essential systems, including reproduction. This can trigger functional hypothalamic amenorrhea. The body slows down its hormone signals to the ovaries. This lowers estrogen and progesterone, which can stop ovulation and cause irregular or missed periods. 

Micronutrient deficiencies and hormonal disruption

By eliminating entire food groups (i.e., fats, grains, dairy, or animal products), orthorexic eating can create significant nutrient deficits that affect reproductive function, including:

  • Essential fatty acids: precursors for sex hormone production
  • Folate and B12: support DNA synthesis and egg quality
  • Iron and zinc: involved in ovulation and follicular development
  • Vitamin D and calcium: support hormone regulation and metabolic health

These deficiencies can contribute to fatigue, hormonal imbalance, and menstrual dysfunction – all of which can make conception harder.

Healthy Eating vs. Orthorexia: How to Spot the Difference

Healthy living exists on a spectrum. Understanding where your mindset and habits land can help you identify when well-meaning behaviors have tipped into orthorexia territory. The table below can help guide you in thinking about how your fertility journey is impacting your relationship with food. This is about looking at how often and/or how many of these indicators resonate with you.

Menstrual cycles act as a free monthly check-up. Irregularities or missing periods can point to chronic under-fueling.

Rebuilding Your Relationship with Food During Your Fertility Journey

Fostering fertility nutrition requires moving beyond what you eat to examine how and why you eat. 

  1. Shift to “Wholesome” Over “Clean”: Replace judgmental labels like “clean” or “dirty” with neutral terms like “wholesome,” “nourishing,” or “satisfying.” Every food (yes, even the ones we label “junk”) can fit into a flexible, healthy lifestyle.
  1. Prioritize Energy Availability: Ensure you eat enough total calories, healthy fats, fiber, and proteins to signal safety to your reproductive system.
  1. Practice Food Flexibility: Intentionally include foods you enjoy without analyzing their ingredient list or micro-nutritional breakdown.

If food choices cause daily anxiety, or if your menstrual cycle has become irregular while trying to conceive, reach out to qualified healthcare providers.

  • Registered Dietitian (RD/RDN): A fertility dietitian specializing in disordered eating can calculate your actual energy needs and build a balanced, non-restrictive meal approach.
  • Reproductive Endocrinologist (REI): A fertility specialist can evaluate your ovulation status, hormone levels, and reproductive anatomy. 
  • Therapist (LMFT, LCSW, or PsyD): A mental health professional specializing in eating disorders or fertility stress can help break rigid thought patterns and restore emotional balance.
